Letter A

 

(Use your company letterhead, fax, and send certified mail)                                                              

 

 

 

To whom it may concern:

 

We hereby notify (your previous payroll company here) that we will no longer be utilizing your payroll and tax filing service effective immediately. Our last check date with your company was (your last check date here).

 

To ensure proper communication with the federal and state payroll tax reporting agencies, please immediately abide by the following course of action for our existing tax liabilities as noted below. We also request your response to this letter in writing, clearly communicating exactly what actions you have taken or will take. Otherwise, failure to respond could lead to our inability to follow the federal and state tax deposit and reporting rules, which would thereby put us in a penalty situation.

 

Please execute the following:

 

Ø      SEND us a report depicting year-to-date and current quarter-to-date totals for
all active, inactive, and terminated employees that received compensation in 2000.

 

Ø      REFUND immediately any open liability for FICA, federal withholding, state withholding, and disability taxes that currently remain undeposited for our account.

 

Ø      REFUND immediately to us any state unemployment insurance (SUI) and federal
unemployment insurance (FUTA) tax that currently remain undeposited for our account.

 

Ø      FURTHERMORE, do not submit the current quarter or any annual payroll tax returns to any federal, state, or local tax agencies.

 

We officially cancel your power of attorney as it applies to any bank or government agencies.

Ø      DEPOSIT immediately any open liability for FICA, federal withholding, state withholding, and disability taxes that currently remain undeposited for our account.

 

Ø      SUBMIT all of our annual payroll tax returns (including W-2's) to the appropriate local, state, and federal agencies.

 

Ø      DEPOSIT immediately any state unemployment insurance (SUI) and federal unemployment insurance (FUTA) tax that currently remain undeposited for our account.

 

We officially cancel your power of attorney as it applies to any bank or government agencies.
